Transitioning from a hospital to a rehab center can be difficult when what you really anticipated upon leaving the hospital was to go home to your own comforts and family members. You can be reassured to know that at Crestwood our number one goal is to prepare you for home by giving you the rehab you need and the care you deserve in the short time you are here with us. We offer both shared and private rooms fully furnished and closet space for your clothing and personal belongings. Bedding and linens are supplied and frequently laundered in-house. We have toiletries and incontinence supplies available. If you prefer to bring your own supplies, you are welcome to pack anything that would make you more comfortable.
You will be assigned to a social worker that will help you to plan the length of your stay by coordinating with your therapists, nurses and the physician to determine when it is best for you to safely return home. The Admission process takes approximately one hour to complete. Information must be gathered, recorded and documents signed. Often it is possible to do this prior to admission into the facility. The admission coordinator will schedule this with you. We are always happy to send someone to a location convenient to you to assist in completing the necessary paperwork prior to admission.
